An elliptical excision is the most severe form of excision. The ellipse is frequently designed in such a way that the consequent scar runs parallel to existing skin wrinkles. This usually results in a wound that is less tense and orients the scar in a less visible way to the eye.

A surgical marker is used to mark the area to be removed. A local anesthetic injection will numb the affected area of skin quickly and keep it numb throughout the procedure. The dermatologist will then use a scalpel and sharp scissors to cut approximately and under the lesion, leaving a suitable margin of normal encircling tissue.
